Client Code of Conduct and Waiver Form:

To enjoy your massage experience to the fullest we ask that you observe the Code of Conduct, and realize that personal awareness can help ensure your satisfaction, comfort and safety, as well as that of others.

 

CLIENTS AND PRACTITIONERS HAVE THE RIGHT TO:

  • A clean, safe, comfortable, and eco-friendly environment.

  • Stop a treatment at any time, for any reason.

  • Be treated with consideration, dignity and respect.

  • Trained staff who respectfully conduct treatments according to State and Holistic Health LMT’s protocols, policies and procedures.

  • Ask about their Holistic Health LMT experience.

  • Information regarding staff training, licensing and certification.

CLIENTS AND PRACTITIONERS HAVE THE RESPONSIBILITY TO:

  • Communicate their preferences, expectations and concerns.

  • Communicate complete and accurate health information and reasons for their visit.

  • Treat staff and other guests with courtesy and respect.

  • Use products, equipment and therapies as directed.

  • Not wear strong/excess perfume or scented body lotion in consideration of our clean air policy and other people's allergies.

  • Adhere to Holistic Health LMT published policies and procedures.

 

-Payment-

**Payment is DUE at the time of service, unless otherwise prepaid.

We accept the following forms of payment: cash, checks, credit card, FSA/HSA Cards,  SpaFinder gift cards as well as gift certificates issued by Holistic Health LMT or an instant gift certificate from our website.  Insurance may cover part or all of your services, please contact your insurance provider and ask about coverage of massage therapy by a licensed massage therapist

*PLEASE NOTE:  To use insurance please bring in your prescription from your doctor or have them email us your script making sure the expiration date is also noted. if you do not currently have a prescription we have forms that may be given to your physician to fill out as an order for Massage.  All paperwork and proceedures must be completed, with a preauthorization from your insurance to insure payment. ** Should your insurance not cover all or part of your care payment will become due at that time and will be fully your responsibility.  We are happy to provide a Superbill for you to submit to insurance for reimbursement.

  •  -Gratuities- Gratuities are always welcome and very much appreciated by our practitioners, although they are not mandatory. Service and gift certificate prices do not include gratuities.  For your convenience, they may be added at time of purchase or when checking out, via cash or credit card.

  • -Prices- Prices are listed on our website and our scheduling site and are subject to change without notice.  Visit www.holistichealthlmt.org and click the “Online Scheduling” link.

  • -Refund Policy- Cash Check or Credit card Refunds will only be given with in 5 days of the purchase date of any product or service.  Any Refund requested after the 5 day refund policy date will be given as a store credit usable for any service or class at any point within a years time.

 

-Making an Appointment-

A valid credit card number or prepayment is needed to hold any massage appointment at Holistic Health LMT.  We recommend reserving an appointment in advance in order to ensure availability; however, we are happy to accommodate you on short notice if we have an opening.

**-Cancelling an Appointment-**

If you need to cancel or change an appointment, please do so at least 24 hours in advance.  There will be a fee assessed to your account if you do not show for your appointment OR call to reschedule or cancel within 24 hours of your designated time.  If you scheduled a same day appointment and must then cancel or reschedule, you will still be charged the full price of your scheduled service as it is less than 24 hours notice.  We strictly adhere to our cancellation policy out of respect for our practitioners as well as our other clients.  We appreciate your dedication.  PLEASE leave a voicemail or text 614-547-WELL (9355) as soon as you realize you may be late or unable to keep your scheduled visit.

Illness/Injury/Family Emergency Related Absence: These are excusable absences and will be taken into consideration for the cancellation of appointments.

-Harassment-

Harassment of any type is not taken lightly and will not be tolerated at Holistic Health LMT.  Our practitioners reserve the right to end a session at any point if they feel uncomfortable with a conversation, certain body movements, or if sexually suggestive comments, questions or behaviors are displayed.  Harassment will result in banning of a client, a fine of $500 and the potential of criminal charges being pressed depending on the severity of the situation.  

  • -Draping-  The State of Ohio requires that all clients be properly draped at all times.  OAC Chapter 4731-26 

 

 -Arrival-

Please be sure to arrive at least 5 minutes prior to your appointment time in order to prepare for your massage experience.  This will give you a chance to unwind and ensure that you start your appointment on time.  If you are running late, please don’t hesitate to give us a call.  Please note that your appointment will still end at its scheduled time and you will still be charged the full amount of your service.  We regret having to shorten any valuable time from your service due to lateness.  Per our cancellation policy, we require a 24 hour minimum notice without a fee.  If no one is available to answer, PLEASE leave a voicemail or text 614-547-9355.

-Environment-

The beauty of silence helps us to create a tranquil environment - please speak quietly during your session.  Please silence all cell phones upon entering the center.  

For the health and safety of all of our clients at Holistic Health LMT, smoking is not permitted inside the facility or within 100 feet of the entrances and exits of our facility.

-Consideration for Clients Who Are Minors-

We give special consideration to clients who are under the age of 18.  Those clients between 1 and 17 years old must present written permission from their parents or guardian prior to their first massage session.  Clients under 15 years old must have a parent or guardian present in the facility at all times.

-Gift Certificates-

Please be sure to treat your gift certificates like cash.  Your gift certificate must be present to be honored.  Please remember gift certificates never include gratuity in the value.

Gift certificate cannot be replaced or refunded for cash value all gift certificates can be exchanged for a different service at Holistic Health LMT

 

-Prepaid Services and Memberships-

Prepaid services including Membership Plans cannot be refunded for cash value.  All sales are final Membership plans must be cancelled at least 14 days prior to the billing date in order to prevent additional billing

-Privacy-

We value your privacy.  We will never give, lease, sell, or otherwise disclose your personal information without express written permission - period.  Any information you give to Holistic Health LMT will be held with the utmost care and will not be used in ways to which you have not consented.

 

Client Waiver-

 

  • I understand that massage therapy is provided for stress reduction, relaxation, relief from muscular tension, and improvement of circulation and energy flow.

  • I understand that by participating in any activity or classes offered at Holistic Health LMT I am required to consent to and sign a waiver 

  • If I experience pain or discomfort during the session, I will immediately inform my therapist so that pressure/strokes can be adjusted to my level of comfort. I will not hold my therapist or Holistic Health LMT responsible for any pain or discomfort I experience during or after the session.

  • I understand that the services offered today are not a substitute for medical care. I understand that my therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.

  • I affirm that I have notified my therapist of all known medical conditions and injuries.

  • I agree to inform the therapist of any changes in my health and medical condition. I understand that there shall be no liability on the therapist’s part nor on the part of Holistic Health LMT should I forget to do so.

  • I understand that massage is entirely therapeutic and non-sexual in nature.

  • By signing this release, I hereby waive and release my therapist and Holistic Health LMT from any and all liability, past, present, and future relating to massage therapy and bodywork.
